This is a temporary position until March 31, 2021.
As part of the Housing Outreach Prototype Pilot (HOPP), workers assertively connect with individuals who chronically sleep rough throughout Edmonton (including in the river valley/parkland areas) to properly prepare them for the housing process and subsequently to provide intensive follow-up support with an overall objective of housing retention.
The Boyle Street HOPP team is multidisciplinary in nature and part of a partnership involving Homeward Trust and the Bissell Centre. Collaboration is a key component of this position.
KEY AREAS OF RESPONSIBILITIES:
· Connect and build rapport with street populations sleeping on public lands through intensive outreach in the River Valley, parks and other public spaces
· Connect individuals sleeping rough to resources that will improve their standard of living, such as income, medical supports, and identification, accompany them to appointments, and advocate on their behalf
· Support clients in their transition from homelessness to being housed
· Maintain detailed case notes, regular assessment, documentation, and files to track outcomes
· Assist clients in accessing household goods and furniture as needed
· Coordinate referrals to other support services such as Home Care, Mental Health professionals, Addiction Treatment, Doctors, Children and Family Services, and others as necessary
· Provide crisis intervention services as needed; including follow-up services until crisis situation has stabilized
· Maintain monthly contact with landlords, and weekly contact with clients.
· Partnership responsibilities including but not limited to: intake, partnership meetings, case conferencing, training and committees.
· Perform other related duties as assigned by the Manager/Team Lead
Education: Degree or equivalent in social services or related field. Candidates without a degree but who have a combination of post-secondary education and related experience will also be considered
Skills, Abilities and Attitudes:
· Able to quickly, creatively and ethically solve problems with clients
· Proven ability to work in a respectful and client-centered manner
· Understands how to use de-escalation techniques
· Works with initiative, commitment and enthusiasm; physically fit
· Good working knowledge of Microsoft Office
· Familiar with entering data into databases, such as the Efforts to Outcomes system
· Able to plan and make decisions in a timely, organized and appropriate manner
· At least two years of experience delivering community-based services
· Knowledge of evidence-based practices, motivational interviewing and cognitive behavioural techniques
· Particular expertise developing and monitoring case plans
· Knowledge of Landlord and Tenant Legislation.
· Excellent knowledge and understanding of poverty-related issues, trends and services, particularly within, but not limited to the urban Indigenous population
· Local travel on behalf of clients and the agency is required; must have a valid class 5 license, reliable vehicle and clean driver’s abstract
· Criminal Record and Child Intervention Checks (results will be reviewed on a case by case basis)
